WHAT IS DIRECT PRIMARY CARE?
Direct Primary Care (DPC) health models allow our patients and physicians to become partners in healthcare decision making while building trust in your doctor. Your doctor will focus on you and your long-term health goals with longer appointments and unlimited medically necessary visits. Our Patients don’t have to worry about co-payments, deductibles, or hidden charges as we are transparent about all fees and your monthly fee covers most primary care services. Our doctor-patient panels are smaller compared to traditional primary care clinics, allowing us to have more time with our patients and less time in administrative work.

DOES SPROUTING HEALTH & WELLNESS ACCEPT INSURANCE?
Currently, we are accepting the insurance plans listed below. We will add insurance plans we accept as we panel with more. When patients opt for insurance use instead of direct primary care plans, the patients agree to all the terms and conditions of their insurance plan, including but not limited to co-pays, deductibles, etc. If you have any questions, please feel free to call our office.

DO I STILL NEED INSURANCE?
We recommend all patients have and maintain insurance coverage for medical catastrophes such as trauma, cancer, and unexpected surgery.
WHAT IF I DON’T HAVE INSURANCE?
Sprouting Health & Wellness will take care of you just like we do with all our patients. No insurance required to come to see us as your primary care physicians.
WHAT IF I NEED CARE ON THE WEEKEND?
We are open on most Saturdays for visits. If you have an urgent after-hours concern that you believe cannot wait for regular business hours, please visit a urgent care facility or the emergency department.
If it is an emergency, please call 911 immediately.
IS THERE COPAYS FOR OFFICE VISITS?
If you are on our Direct Primary Care memberships there are no copays to worry about because you pay a low monthly fee that covers most primary care visits*. There are copays for patients that use their insurance plan and are subject to health insurance terms and policies.
CAN I SUBMIT CHARGES TO MY INSURANCE COMPANY?
Not if you are using the direct primary care plan. If you are using your health insurance, your insurance and you will be billed as per the terms of your insurance policy.
CAN I BENEFIT FROM SPROUTING HEALTH & WELLNESS IF I DON’T REQUIRE FREQUENT MEDICAL ATTENTION?
Yes. We always provide care for patients that are sick, but it is important to remember that being proactive helps keep you healthy. At Sprouting Health & Wellness we work with you to tailor healthy lifestyle plans that fit your needs.
ARE LABS AND IMAGING COVERED?
No, those are out-of-pocket expenses for the direct primary care plans. We have negotiated discounted pricing for our members. We will never perform any services without fully discussing costs with you.
If you are using your health insurance plan, your insurance policy and you will be billed according to the terms of your health plan.
